Copy the Helvetica fonts located in the Fonts folder to your system�s Fonts directory. Typically, that is C:\Windows\Fonts
Open in Photoshop 01 Front and your photo.
Edit Name, Sex, Date of Birth, and other desired text items.

Edit Signature or delete Signature and scan your signature into a file and drag into position.
Make the background of your signature file transparent.
Delete Photo layer. Then highlight Background layer.

Drag your photo onto and into position. Your photo will be Layer 1 and should slide between the Background layer and the Overlay layer

Save and close 01 Front.
If desired, open 02 Back and edit thumbprint and save.
